Please complete and submit this form. Your supervisor/student advisor will then receive an automatic email at the address you give under Supervisor Email, asking them to confirm your student status. If you apply for a fee waiver, they will also be asked to confirm you are eligible for this (i.e. you do not have access to financial support for membership/conference fees). Your advisor's response will then be manually processed by the Association's administrator, after which you will receive by email a link for completing your membership application.
